Dana spent nine years processing claims for a regional hospital before opening Heartland. Most of what the agency does differently comes out of that: the questions are about your doctors and your medications first, because those are what actually determined whether a claim was paid.

She takes the households with complicated income — self-employment, seasonal farm income, a spouse with employer coverage — which are the ones the estimate goes wrong for.
